Accurate directory information matters. If you find a possible problem with a public provider record, you can report it for review without sending private health information.

Four steps for reporting a listing concern

1. Identify the record: include the provider name, NPI if available, and the SimplyFind page link.

2. Describe the concern: state clearly what may be inaccurate, outdated, or incomplete.

3. Share official supporting context where available: for example, a public source link or a corrected practice detail. Do not send private medical information.

4. We review the report: the directory team reviews the information and may update the display when appropriate.

Send a correction request

Email support@simplyfind.co with the provider name, NPI if available, page link, and a concise description of the issue. Providers should also correct NPPES information where applicable, because NPPES is the underlying public source record.

Please do not include diagnoses, treatment details, insurance numbers, appointment information, or other sensitive health information.
